Abstract. Non-Communicable Diseases (NCDs) are the leading cause of death in Indonesia, with a continuously increasing prevalence trend. Early detection and self-monitoring of NCD risk factors are highly needed, especially among the productive age population. This community service research aims to provide assistance in using the Adem-Gluacho application as a digital medium for health monitoring in Banjarsari Village, Cerme District, Gresik Regency. The method used consists of five stages: preparation, socialization, training and mentoring, application trial, and evaluation. The participants numbered 70 people aged 25–55 years who met the inclusion criteria. The results showed significant improvements in three aspects: health knowledge (20%), technology knowledge (20%), and technology skills (35%). The majority of participants who had never used a health application before were able to record their self-examination results through the application. Thus, the implementation of the Adem-Gluacho application proved effective as a digital innovation to support recording, monitoring, and early detection of NCD risk factors in the community.
Abstrak. Penyakit Tidak Menular (PTM) merupakan penyebab utama kematian di Indonesia dengan tren prevalensi yang terus meningkat. Upaya deteksi dini dan pemantauan mandiri faktor risiko PTM sangat diperlukan, khususnya pada masyarakat usia produktif. Penelitian pengabdian masyarakat ini bertujuan untuk memberikan pendampingan penggunaan aplikasi Adem-Gluacho sebagai media digital pemantauan kesehatan di Desa Banjarsari Kecamatan Cerme Kabupaten Gresik. Metode yang digunakan meliputi lima tahapan, yaitu persiapan, sosialisasi, pelatihan dan pendampingan, uji coba aplikasi, serta evaluasi. Peserta kegiatan berjumlah 70 orang berusia 25–55 tahun yang memenuhi kriteria inklusi. Hasil kegiatan menunjukkan peningkatan signifikan pada tiga aspek, yaitu pengetahuan kesehatan (20%), pengetahuan teknologi (20%), dan keterampilan teknologi (35%). Mayoritas peserta yang sebelumnya belum pernah menggunakan aplikasi kesehatan berhasil melakukan pencatatan hasil pemeriksaan mandiri melalui aplikasi. Dengan demikian, penerapan aplikasi Adem-Gluacho terbukti efektif sebagai inovasi digital dalam mendukung pencatatan, pemantauan, serta deteksi dini faktor risiko PTM di masyarakat.
