Digitalisasi Kesehatan untuk Pemantauan Penyakit Tidak Menular (PTM) Kelurahan Kalirungkut Kecamatan Rungkut Kota Surabaya

Penyakit Tidak Menular (PTM) seperti hipertensi, diabetes, dan kolesterol tinggi merupakan penyebab utama kematian di Indonesia. Berdasarkan Profil Kesehatan Surabaya tahun 2022, prevalensi PTM terus meningkat, sehingga diperlukan langkah preventif dan monitoring secara berkala. Kegiatan pengabdian masyarakat ini berfokus pada pemantauan PTM di Kelurahan Kalirungkut, Kecamatan Rungkut, Surabaya melalui digitalisasi kesehatan. Kegiatan ini memanfaatkan aplikasi Adem-Gluacho, yang dikembangkan untuk memudahkan masyarakat dalam memantau kondisi kesehatan mereka. Aplikasi ini memiliki fitur untuk mencatat hasil pemeriksaan gula darah, asam urat, dan kolesterol, serta menyediakan konsultasi berdasarkan hasil pemeriksaan tersebut. Pelaksanaan kegiatan terdiri dari tahapan perencanaan, sosialisasi kesehatan, pelatihan penggunaan aplikasi, dan pendampingan penerapan teknologi. Program ini melibatkan warga berusia 25 tahun ke atas dengan kategori sehat, berisiko, dan penyandang PTM. Hasil kegiatan menunjukkan bahwa warga sangat antusias, terbukti dengan tingginya partisipasi dan minat untuk menggunakan aplikasi Adem-Gluacho. Pemanfaatan teknologi digital ini berhasil meningkatkan frekuensi pemantauan kesehatan mandiri oleh masyarakat, serta meningkatkan pemahaman warga terkait pentingnya pemantauan dan pencegahan dini PTM. Kegiatan ini diharapkan dapat menjadi model untuk penerapan program serupa di wilayah lain guna meningkatkan kualitas kesehatan masyarakat secara berkelanjutan.
